Connect Programme
The CONNECT programme is for a person that needs support after a period of treatment or at a stage in life where you feel that you need extra support and typically lasts for 4-12 weeks. A person can receive help and support in the privacy of their own home or anywhere in the world.
The CONNECT programme will help you to feel supported, achieve your goals, offer you emotional strength and spiritual guidance. A person may need support after a period of treatment, hospital, crisis, relationship issues or are at a stage in life where you just feel you need some extra support. Whether you are experiencing, anxiety, depression, anger, substance misuse or needing help with maintaining your recovery our CONNECT programme can help.
The purpose of the CONNECT programme is to help you move forward from any self defeating behaviour, by helping you to make healthy choices about your lifestyle, activities and relationships. This solution focussed approach will help to empower you and thus increase your self- efficacy and esteem. A person will have a life coach/therapist for two sessions a week. The first session will be an assessment and a care plan formulated with the specific goals the person would like to achieve. Our life coach/therapist will also provide daily support via sms and inspirational relevant topics. You will be set initial and ongoing personal goals which will be reviewed as part of your work.
In some cases, we also use app technology and this can assist in helping you to be accountable to your agreed treatment plan. Our life coach/therapist will track your progress via the app and be able to see what is going well and when you are in danger of returning back to any self defeating patterns of behaviour.
The programme consists of:
Assessment
Care plan
Life Coaching - Skype or Phone calls twice a week
Goals Setting and Worksheet
Relapse Prevention Skills (if necessary)
App Technology
Daily SMS Messages
